Injured at work?

If you have been injured on the job:

• Report the incident to your employer or supervisor immediately. Fill out an incident form to document it happened and to help your employer look for ways to prevent it from happening again.
• Seek treatment, if necessary. Your employer can help you get appropriate medical treatment, but you have the right to select your own doctor. Let your employer know who is treating you; your employer can work with your medical provider to get you back to work. (An attending physician or authorized nurse practitioner must be able to verify that there are objective findings of an injury or disease.) Types of injuries: An injury can be traumatic (caused by an accident), cumulative (caused by repetitive motion), or an occupational disease (such as loss of hearing).
• File a workers' comp claim with SAIF. The best way to file a claim is to complete the standard claim form, called the 801 form. It's available in these formats:

Note: If you are a longshoreman, you'll need to fill out the Federal Longshore LS-202 form. 

If you are in the hospital or cannot complete the form due to injury, your employer can send your form to SAIF without your signature.

Your employer should give you a copy of the completed form and a copy of "A Guide for Workers Recently Hurt on the Job", which provides information on benefits.

• You can also establish a claim by filling out a first medical report (form 827) with your medical provider's office. Be sure to tell your medical provider you have filed a workers' comp claim.
  
• You will be eligible for the First Fill pharmacy program, which provides a limited number of cost-effective prescription drugs for work-related injuries or illnesses until a decision has been made on your claim.
  
  Your pharmacy bills will be handled through the Oregon Prescription Drug Program (OPDP) and paid by SAIF, even if the claim is later denied. There will be no out-of-pocket costs to you for the prescriptions, unless a medication not covered by First Fill is prescribed.
