Filing a claim

If you have a work-related injury requiring medical treatment and you wish to file a workers' compensation claim, here's how to ensure your claim is processed correctly:
  • Contact the claim filing administrator at 888.365.0001 or 503.378.3099 and tell them you are a home care worker and want to file a claim. They will help you get the claim started and sent to SAIF.
  • Tell your client and case manager immediately if you believe you were injured on the job.
  • If you seek medical treatment for your work injury, tell the doctor you were hurt on the job. Complete the Attending Physician form (827). On the form be sure to identify yourself as a home care worker.
  • To learn more about what do to if you're injured on the job, download one of the following brochures:

If you cannot reach the administrator at the above number, call SAIF at 800.285.8525 and ask us to mail you an 801 claim form (English, English with form fields, Spanish, or Spanish with form fields). On the form, identify yourself as a home care worker.

Important

  • Filing a claim does not automatically qualify you for workers' compensation benefits.
  • Deliberately making false or misleading statements for the purpose of obtaining any benefit or payment is considered fraud and is punishable by law.
