Standard billing forms and NPI update
SAIF accepts only the new versions of the CMS-1500 and UB04
CMS has updated the forms used to bill non-hospital services (CMS-1500) and hospital services (now the UB04).
Many insurers are now requiring the use of provider's National Provider Identifier (NPI). SAIF also requires the NPI on both forms. SAIF still requires the provider's Tax Identification Number (TIN) on all billings submitted for payment.

Billing tip: When faxing a bill on the new CMS1500, please be aware that the text on the shaded lines in Field 24 may not come through on the fax. The shading overshadows the printing, leaving it unreadable in some cases.
